We are seeking a skilled and compassionate Registered Nurse (RN) to join our hospice care team. The RN Care Manager plays a key role in assessing, planning, coordinating, and delivering high-quality care to patients in the community setting. This role is ideal for nurses who thrive in a collaborative environment focused on patient advocacy, clinical excellence, and continuity of care.
\n\n\n Primary Responsibilities \n\nPerform comprehensive assessments of patients with complex or high-risk needs.
\n\t\n\tDevelop and implement individualized care plans tailored to each patient’s goals and medical conditions.
\n\t\n\tCollaborate with physicians, social workers, discharge planners, and other care providers to ensure a coordinated approach to patient care.
\n\t\n\tFacilitate smooth care transitions between settings and providers, ensuring continuity and quality of care.
\n\t\n\tEvaluate and adjust care plans to maintain cost-effective and appropriate levels of care.
\n\t\n\tIdentify and address social determinants of health such as housing, transportation, and community support.
\n\t\n\tPromote wellness and illness prevention strategies.
\n\t\n\tMaintain detailed and accurate records of all clinical, functional, and fiscal activities.
\n\t\n\tServe as a patient advocate while upholding privacy and confidentiality standards.
\n\t\nAssociate’s or Bachelor’s Degree in Nursing (BSN preferred).
\n\t\n\tActive and unrestricted Colorado RN License or Compact Multi-State License .
\n\t\n\tPrior experience in Hospice Care or Home Health setting preferred.
\n\t\n\tHCHB (HomeCare HomeBase) experience strongly preferred.
\n\t\n\tReliable transportation and valid auto insurance required.
\n\t\n\tStrong critical-thinking, communication, and decision-making skills.
\n\t\n\tCase Management Certification (CCM or equivalent) preferred.
\n\t\nIDR Job ID #468479. Pay package is based on 8 hour shifts and 40 hours per week (subject to confirmation) with tax-free stipend amount to be determined.
