IDR Healthcare is seeking a travel nurse RN Case Manager, Hospice for a travel nursing job in York, Pennsylvania.

\n Shift Type: Days | No weekends | No on-call required
We are seeking experienced and compassionate Registered Nurses (RNs) to join our hospice care team. The RN will provide high-quality clinical care to patients receiving end-of-life services, ensuring dignity, comfort, and support for both patients and their families.
\n\nCandidates must be comfortable working independently, managing a daily caseload, and delivering comprehensive nursing assessments and interventions across a variety of care settings.
\n\n\n Responsibilities \n\nPerform comprehensive nursing assessments including admissions, recertifications, routine, PRN, and death visits.
\n\t\n\tDevelop, implement, and evaluate individualized care plans in collaboration with the interdisciplinary team.
\n\t\n\tEnsure quality and compliance through accurate and timely documentation in the EMR system (HomeCare Homebase experience strongly preferred).
\n\t\n\tMaintain productivity benchmarks of 5–7 visits per day while delivering compassionate, patient-centered care.
\n\t\n\tCoordinate with physicians, social workers, and other healthcare professionals to facilitate care transitions and appropriate levels of service.
\n\t\n\tIdentify and address social determinants of health that impact patient care (e.g., housing, transportation, or community resources).
\n\t\n\tPromote health education, preventive care, and emotional support for patients and families.
\n\t\n\tAdvocate for patient rights while maintaining confidentiality and ethical standards.
\n\t\n\tParticipate in interdisciplinary team meetings and contribute to continuous quality improvement initiatives.
\n\t\nActive RN license in good standing (state-specific).
\n\t\n\tCurrent BLS/CPR certification required.
\n\t\n\tValid driver’s license and auto insurance.
\n\t\n\tHospice experience required; HomeCare Homebase (HCHB) proficiency strongly preferred.
\n\t\n\tAssociate’s or Bachelor’s degree in Nursing (BSN preferred).
\n\t\n\tStrong clinical judgment and critical-thinking skills in complex situations.
\n\t\n\tExceptional communication and organizational abilities.
\n\t\n\tCase Management certification preferred, but not required.
\n\t\nCompassionate and patient-focused approach
\n\t\n\tExcellent time management and self-motivation
\n\t\n\tAbility to function effectively in a field-based or home-care environment
\n\t\n\tCommitment to interdisciplinary collaboration and quality outcomes
\n\t\nIDR Job ID #467511. Pay package is based on 8 hour shifts and 40 hours per week (subject to confirmation) with tax-free stipend amount to be determined.
