Po sition Summary

We are seeking a compassionate and detail-oriented Case Manager to join our team. This role focuses on coordinating patient care, supporting treatment plans, and improving health outcomes within a fast-paced healthcare environment.

Key Responsibilities

• Assess patient needs and develop individualized care plans.
• Coordinate with providers across departments, including ICU and PICU.
• Maintain accurate documentation and medical records.
• Provide education and resources to patients and families.
• Support smooth transitions of care and discharge planning.
• Monitor patient progress and adjust care plans as needed.
• Advocate for patients to ensure timely access to services.

Qualifications & Skills

• Strong knowledge of patient services and case management processes.
• Familiarity with pediatrics, trauma care, or acute care settings preferred.
• Proficiency in medical terminology and documentation.
• Excellent communication and teamwork skills.
• Ability to thrive in high-pressure environments.
• Strong organizational and time-management abilities.
• Compassion, empathy, and professionalism in patient interactions.

Schedule & Work Location

• Full-time, 8-hour shift
• In-person position
