Overview Ramsey County Human Services Overview: Human Services
to residents to help them meet their basic needs and chieve their highest potential. These services include but are not limited to:
- Ensuring the safety of children and vulnerable adults and helping those in crisis.
- Helping people meet their financial, nutritional, health care, and social support needs.
- Securing nd maintaining safe and supportive housing.
Human services are provided to residents through various programs targeted toward specific needs or populations, each of which has eligibility criteria.
In Minnesota, the legislature delegates administration of many human services programs to counties who work under supervision from the Minnesota Client (Client). In this state-supervised and county administered model, the state outlines required policy rules state technologies for use in support human services. Program functions performed by counties in dministering human services include screening, intake, investigations, eligibility determination, service planning, nd case management.
Within Ramsey County, human services are coordinated and delivered by multiple departments, including the Social Services Department (SSD), the sponsor of the Traverse implementation.
Human Services Modernization (HS Mod) Program and Traverse Established in 2022, the Ramsey County HS Mod Program identified and initiated several technology improvements to better support residents and focus outcomes within 3 main areas:
- Workflow
- Improving processes for the benefits of residents and staff.
- Dat
- Improving data management and access for workers to enable more efficient response to residents.
- Legacy Modernization
- Improving overall resident and staff experience to access and utilize information and services effectively, efficiently, nd equitably.
Early in the program, electronic case file management was recognized as an opportunity to reduce paper files and improve workflow and data management. In 2023, Ramsey County SSD identified the Northwoods Traverse tool as their preferred option. During 2024, the HS Mod Program team began high-level requirements gathering and contracting.
mong Ramsey County's 542,000 residents, one in three residents (individuals and families) receive human services. In 2025, the Traverse implementation will focus on moving SSD from paper to electronic case file management, allowing the department to continue providing the most basic needs of food, income, shelter, health, and wellness, supporting our residents to not only survive, but thrive.
HS Mod SSD Traverse Implementation Project
Business Need Ramsey County SSD currently uses paper-based files to manage resident cases. Electronic case file management is needed to improve case, file, form, document management and workflow, and improve resident support through modern technology and updated processes
Project Overview, Scope, nd Expected Benefits Project Overview The Traverse electronic case file management tool will replace paper case files with a tool to create and manage digital case files. Doing so enables complex decision-making nd robust reporting for SSD. Project kick-off is expected to occur in January 2025, with completion before December 2025. The implementation is a large project encompassing multiple workstreams including but not limited to technology, business requirements, training, and change management. The project will be led by a program manager with multiple project managers facilitating workstreams.
The project team will collaborate with Northwoods Consulting and multiple business teams to develop a co-owned project plan and project documents. Project managers are expected to have strong facilitation, documentation, and business problem solving skills, and the ability to manage multiple work streams within the project. In addition to managing workstream project documents and facilitating meetings, they support the project by assessing needs and documenting options, managing requirements gathering, and creating various project documents.
Project Scope Scope includes project initiation, requirements gathering, planning, technical, implementation, change management, training, implementation and Traverse post-implementation support transition to operations and IS teams. Post implementation, Traverse will:
- Integrate electronic forms and documents.
- Provide automated workflow for internal and external collaboration and document availability and management.
- Improve case file access and management.

Primary Responsibilities and Deliverables - Understands the Project drivers and expected business outcomes.
- Develops the project management plans in collaboration with key stakeholders including the vendor to chieve business outcomes.
- Facilitates the day-to-day spects of Project schedule, scope, nd quality.
- Collaborates s necessary to elicit, document, prioritize, nd follow through to completion business and solution- related requirements.
- Develops nd coordinates pplication support training with the vendor.
- Logs nd follows through to disposition Project-related risks, ction items, issues, nd decisions.
- Performs risk management; escalates issues and risks as needed.
- Provides complete transparency to project performance.
- Controls scope to achieve value within the approved time nd cost; recommends timely corrective actions.
- Collaborates to develop workstream transition plan.
- Monitors progress nd reports status.

Candidate Qualifications Mandatory Qualifications: - B.S. or B.A. degree (4 year)
- 5+ years experience as senior project manager including n pplication implementation
- Demonstrated bility to elicit and document business requirements nd create process flows

- LOCAL to the Twin Cities metro area
Desired Qualifications: - Experience managing technology projects in state, county, or city governments or non-profit sector
- PMP certification from PMI, or other Project management certification
- Demonstrated competency in small group facilitation nd business relationship management

Project Environment and Schedule Project Environment : - Ramsey County is committed to racial equity, diversity and inclusion nd the prevention of workplace violence.
- The Project is sponsored by the EPMO, division within the Enterprise and Administrative Department, in the Information nd Public Records Service Team.
- The consultant resource for this position will be managed by the Enterprise Project Management Office.
- The Project Manager consultant will report to a Program Manager and work with program team members including sponsors, functional leads, technical resources, external staff and departmental subject matter experts. Work will be primarily conducted during standard business hours. The resource is required to be onsite as required. There are no " set days " to be on site; rather, the requirement to be onsite is on an " s needed " basis.
- Ramsey County is operating in a flexible workplace environment. This means that Ramsey County employees work primarily off-site, but in-office work may be required from time to time.

- Estimated Hours: 15-25 hours on average per week
- The County will retain the option to extend the work orders in increments as determined by County staff.
