Position Summary:

We are seeking an experienced and detail-oriented MDS Nurse to join our nursing team. The MDS Nurse is responsible for accurately completing and submitting Minimum Data Set (MDS) assessments in accordance with state and federal regulations. The ideal candidate has strong clinical assessment skills, excellent knowledge of Medicare/Medicaid regulations, and a passion for improving resident outcomes.

Key Responsibilities:

• Complete accurate and timely MDS assessments and care plans for residents.
• Coordinate with interdisciplinary team members to ensure comprehensive assessments and individualized care planning.
• Monitor triggers and quality measures to ensure compliance and quality care.
• Review documentation to support RUG/PDPM reimbursement levels.
• Participate in care conferences and audits as needed.
• Ensure assessments are submitted accurately and on time to CMS.
• Stay current with regulatory updates and ensure facility compliance.

Qualifications:

• Licensed LVN in CA.
• Minimum of 1 year experience with MDS 3.0 in a long-term care setting.
• Strong knowledge of Medicare/Medicaid guidelines and PDPM.
• Proficient with EMR and MDS software (e.g., PointClickCare, MatrixCare).
• Excellent communication, organizational, and critical thinking skills.
• Ability to work independently and as part of a team.
