World Nurses is seeking a LPN / LVN Long Term Care for a local contract job in Honolulu, Hawaii.

Job Summary

We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our team in a Long-Term Care/Skilled Nursing Facility setting. The ideal candidate will provide high-quality nursing care, promote patient well-being, and support residents in maintaining the highest level of independence, dignity, and comfort. This role offers the opportunity to make a meaningful impact every day while working in a supportive care environment in beautiful Honolulu, HI.

Key Responsibilities

• Provide direct patient care in accordance with established nursing standards and facility policies.
• Assist with assessments, care planning, and implementation of nursing interventions.
• Administer medications and treatments as prescribed.
• Monitor, document, and report changes in resident conditions to the RN or physician.
• Support activities of daily living, ensuring safety, dignity, and resident satisfaction.
• Collaborate with interdisciplinary teams including RNs, CNAs, therapists, and families.
• Maintain accurate documentation and compliance with state and federal regulations.
• Participate in care conferences, training, and quality improvement initiatives.

Qualifications

• Active LPN license in the state of Hawaii (or eligibility to obtain).
• Previous experience in Long-Term Care or Skilled Nursing preferred.
• Strong communication, teamwork, and organizational skills.
• Ability to work in a fast-paced, resident-focused environment.
• Compassionate approach and commitment to quality care.

Preferred Skills

• Familiarity with EMR charting systems.
• Experience supporting geriatric populations.
• Knowledge of state and federal LTC regulatory standards.
