PRIDE Health is seeking a LPN / LVN Case Management for a local contract job in Dallas, Texas.

Pride Health is hiring a Case Manager LPN for our client in Dallas, TX! 

Key Responsibilities:

• Conduct initial interviews and Chronic Care Model visits.
• Educate patients on chronic diseases (COPD, CHF, diabetes, IHD).
• Use motivational interviewing to support lifestyle changes.
• Help patients set and track self-management goals.
• Coordinate follow-up care and community resources.
• Document visits and progress in the electronic medical record.
• Maintain knowledge of chronic disease treatments and medications.

Qualifications:

• High school diploma or GED.
• Current LVN license in state of employment.
• 2+ years of clinical experience.
• Experience in cardiac, med-surg, or critical care settings.
• Knowledge of chronic conditions and patient education.
• Basic computer skills and EMR documentation.
• Valid driver’s license and reliable transportation.
• Must meet immunization and TB screening requirements.

Preferred:

• 5+ years of clinical experience.
• Bilingual (Spanish).
• Knowledge of managed care and medical coding.
• Strong communication, organizational, and teamwork skills.
