As a Medical Provider at Horizon, you will…

  • Provide initial addiction medicine assessments for individuals admitted to substance use, rehabilitation and recovery programs and provide follow up medication management and evaluation services.
  • Participate in clinical case conference and case reviews to programs to provide consultation and guidance with difficult cases, assures communication in multiple forums with counseling staff to be updated on consumer progress and outstanding issues or concerns.
  • Prescribe appropriate medication for substance use disorders.
  • Communicate to primary counselor the need to make referrals for physical exams with consumer's primary care physician and specialists.
  • Ensure that all confidential and potentially sensitive information is processed, maintained, and utilized in a strictly confidential manner.
  • Document all services provided in the case record. Must be literate with electronic health records.

What makes you a great candidate?

We can’t wait to learn more about you! Here are a few specifics of what you’ll need for the job:

  • Nurse Practitioner NY state license, certified and eligible for credentialing with all insurance payers.
  • 2 years’ experience preferred, specifically with individuals with substance use needs.

Location :

This position would be located at 77 Broadway, Buffalo NY. There may be times when working at multiple Horizon locations is required.

Hours:

This is a full-time position with flexible hours from Monday - Friday.

Compensation:

Horizon is consistently evaluating our compensation and benefit packages to ensure we remain competitive. The range of pay for this position is $65.00 - $71.50 per hour. Starting pay may be impacted based on experience and education. This position includes a bonus earning opportunity, with an annual max potential of $2,400, based on volume of service visits provided and cash equivalent opt out benefits. Compensation adjustments are evaluated annually.

Physical demands associated with this position include:

  • Stationary posture with frequent standing/sitting alternation.
  • Mobility between nearby worksites and within facilities.
  • Mobility required involving repetitive wrist, hand, and finger movements.
  • Lifting objects up to 20 pounds.
